Online Member Application
If you have any questions about applying to UUSCM, please don't hesitate to contact us at email@example.com.
Click on the link below to fill out the online application. (It's a simple Google form.) When you've filled out the application. it will be posted to the Administrator, who will send you a confirmation email and contact your references. Please tell the persons you're asking to serve as references that they should expect to hear from us.
After we hear from your references, we'll send you an invoice for the coming year's dues. As soon as you pay your dues, your membership will be activated. Once you are an official member you will be able to log in and view the Members Only area. You can also modify any of your information and set Privacy Settings.
Our default Privacy Settings are that the General Public can view your professional information and members can see your personal information. However, you may use the Privacy Settings to reset those defaults to keep any of your information private. If you have a particular issue, email us for help.
Your membership will be due for renewal each year at the same date that you joined.
